HIV CARE COORDINATOR
Position Summary
Callen-Lorde is seeking a dedicated, flexible individual to join our staff as an HIV Care Coordinator. The HIV Care Coordinator is responsible for ensuring the continuity of HIV primary care including assessment, treatment plan development, navigation of medical and support services, regular plan updates, and re-evaluation. The HIV Care Coordinator will assist patients, or provide external referrals, to address barriers to accessing medical care, including health insurance, benefits, entitlements, housing, and supportive services. The HIV Care Coordinator will also be responsible for oversight of patient navigators who will provide field case management services to patients. The Care Coordinator will work in collaboration with medical providers, nurses, mental health staff, and prevention and outreach providers to ensure that the full continuum of care needs for our HIV-positive patients are identified and appropriate services accessed, coordinated, and monitored.
Work Schedule/Salary/Benefits
This position is scheduled Monday through Friday. Some evening hours will be required. Required Qualifications
  • Master's degree in health/social services/psychology or related field.
  • 2 years experience in HIV case management.
Preferred Qualifications
  • Experience working with gay and/or bisexual men.
  • Experience in HIV/AIDS social services field.
  • Knowledge about HIV/AIDS.
  • Bilingual (Spanish/English).

PATIENT NAVIGATOR
Position Summary
Callen-Lorde is seeking a dedicated, flexible individual to join our staff as a Patient Navigator. The Patient Navigator is responsible for providing field-based case management and adherence support services to HIV-positive individuals accessing medical services. The Patient Navigator will work in a team setting headed by a Care Coordinator and in conjunction with medical and mental health providers to ensure that patients maintain stable health. This is a field-based position, working with patients in their homes and accompanying them to appointments, and as such, the schedule is determined by patient needs. The goals of patient navigation are to ensure that HIV-positive patients are linked to care in a timely manner, to maintain patients in care through coordination of medical and social services, and to provide support and coaching. By teaching and supporting treatment/medication adherence, the Patient Navigator will help patients become self-sufficient and able to manage their medical and social needs autonomously.

DENTAL ASSISTANT
Position Summary
Callen-Lorde is seeking exceptionally friendly, organized individuals with excellent skills to join our staff as Dental Assistants. In this position, you will provide clinical support to dentists and hygienists, including preparation of patients, supplies, and rooms, chair-side assistance, and radiology and laboratory assistance. Additionally, you will be responsible for ensuring appropriate and efficient patient flow for the Dental providers.

Position Summary
Callen-Lorde is seeking an exceptionally friendly and organized individual with computer proficiency who is dedicated to serving the needs of the LGBT community to join our staff as a Patient Care Associate (PCA). The PCA is part of our dynamic administrative services team and works in collaboration with our Oral Health staff to ensure optimal patient care. The PCA is responsible for maintaining an efficient flow of patients and phone calls into the Dental Clinic, providing administrative support to the clinical staff as well as assisting with the collection of patient registration, billing, and payment data. The PCA is the primary customer service contact with our patients and must be courteous, patient, and professional at all times.

Position Summary
The Licensed Practical Nurse is responsible for assisting primary care providers, physicians, and mid-level providers by providing general nursing care, patient screening, and clinical support. This position reports to the Associate Director of Nursing.

Position Summary

MEDICAL PROVIDERS
Licensed/certified volunteers, providing direct patient care services, must be credentialed by the relevant Callen-Lorde department. Callen-Lorde welcomes qualified volunteers in the following positions: MDs, NPs, PAs, DOs. Must be NYS licensed and maintain individual malpractice coverage.

Callen-Lorde volunteer medical providers provide episodic care for patients enrolled in adult primary care services. In addition, some providers choose to work within the Sexual Health Clinic, which provides a comprehensive, sex-positive approach to screening, diagnosis, treatment, and prevention of recurrence of sexually transmitted infections (STIs) involving both medical and counseling interventions. All medical provider volunteers work Wednesday evenings under the supervision of a staff physician.
